Customising email addresses MS SBS 2003

Looking after a number of SMB’s running Microsoft Small Business Server 2003 means that often they want customised email addresses.
For example CompanyA might want the default email address to be firstname@example.com but CompanyB wants firstname.lastname@example.com and of course there are endless other possibilities.
